Louisiana Homes I and II are two highly individualized, staff intensive, 24-hour care programs, both functioning in a residential home-like setting.
LHI is designed for clients who require supervision and support.
DESIGN:
Our purpose is to stabilize presenting symptoms through individualized treatment modalities and to provide person-centered, family-focused, and culturally competent services in conjunction with our ancillary service providers. The expected length of stay is determined by clinical need, medical necessity, and reunification with family.
SERVICE MODEL:
We serve with social beliefs and collaborative behaviors. This is accomplished by modeling consistency and trustworthiness throughout the entire milieu.
Our ancillary interdisciplinary team develops and updates an Individual Plan of Service for each resident and is modified annually or more frequently to support the client's needs.
Louisiana Homes believes that behavior can be changed through treatment and supportive services. Without such change, difficulty and disruption occurs for the person served, and for his family, friends, and the community.
We believe that treatment and service delivery requires a comprehensive, yet individualized effort for each person served.
Therefore, services have been developed to meet the specific treatment and rehabilitation needs of each person served.
Louisiana Homes, Inc. assures that the resources (e.g., personnel, funding, space, supplies) needed to provide the services offered in each of its programs are available.
Louisiana Homes was founded in October 1979 to provide a supportive residential program for adults with chronic and severe mental illness.
